The NOHCCN was established to create a platform of united voices for Home & Community Care front-line teams, which covers the Thunder Bay, Sioux Lookout and Moose Factory zones, for First Nation Communities that have Home & Community Care Programs. See our interactive map.

Who We Are

The Northern Ontario Home and Community Care Network is comprised of seven seats of experienced home care providers from various organizations and areas of Northern Ontario. The Board meets regularly to oversee our NOHCCN activites.

Our Mission

The NOHCCN fosters knowledge exchange and transfer of new ideas, supports communities to build capacity and access resources, and advocates for future home and community care needs.
